All you need to do is send a note to us at

giving us the following information

a) Your IANTD Diver Number (if known) & Date of Birth
b) Course Level
c) Date completed
d) Instructor and Facility
e) Where it took place
 f) Your present mailing address

and enclosing credit card details or a cheque for £10.00 payable to IANTD Merchandise Ltd.

You may also send a passport photo if you want a picture on your card(s), (although this is no longer required).
